  • How early should I arrive before my appointment? What should I expect?

    When you schedule your appointment, our staff will inform you when you should arrive for your visit, what you should expect, and how you should prepare for it. To expedite your registration, you can complete our forms and bring them with you.

  • What should I bring to my initial appointment?

    You should bring copies of all pertinent medical records (physicians’ notes, laboratory tests), and imaging results, including actual films. This information is important to help your physician understand your medical history as well as diagnose and offer treatment options. You should also bring your Health Care card and referral. Prior to your visit, you will be sent a registration packet that should be filled out. Please complete and return this information prior to your visit so we can update your information in our new computer system. We recommend that you arrive at least 15 minutes before your scheduled appointment time to complete any remaining paperwork.

  • What do I do if I run out of my medications?

    Your physician can mail or call in prescription refills ONLY if that physician provided the original prescription form. If this is the case, please call us at (289) 803-2959.

  • How can I get a copy of my test results and medical records?

    To receive a copy of your medical records or test results, please call (289) 803-2959. You may be required to sign a consent form for release of medical information if records are to be sent to another hospital or office.

  • How will I pay for tests?

    All services are covered by OHIP – Ontario Health Insurance Plan.
